Report Your Innovation

Submitting a report is the first step in a process that could potentially lead to the commercialization of your technology.

We have updated our forms as of July 2019 (see below). Our new forms feature:

• New names.  To better align with Federal reporting requirements and guidelines, we are re-naming our three disclosure forms (invention, software, and mobile app) to reports.
• Improved formatting
• Expanded space for inventor/contributor information   
• Some new questions that will help us evaluate, protect, and potentially commercialize the intellectual property (although you do not have to answer every question to submit a report).

If you have any questions, or would like assistance completing the form, please contact us. Reports can be submitted to OTM in person or electronically. In addition, please distribute copies of the report to each inventor and unit executive officer. Upon receipt of a report, the OTM will assign it to a technology manager who will contact you.
